Working with OPT is easy!
Here’s how to get started and what you’ll need to bring.

To ask a question:
Same thing! Just give us a call and we’ll do our best to answer any questions you may have.
Insurance
We’re contracted with most insurance plans. As a courtesy, we’ll even contact your insurance company prior to your first visit to obtain a quote of your benefits. Keep in mind, though, that we cannot guarantee the verbal quotes that we receive. For the best estimate of your benefits, we recommend that you also contact your insurance company directly to inquire about coverage for physical therapy in an office setting. You may also refer to your written insurance plan summary for assistance.
Your first appointment
Please arrive 30 minutes early, and expect the initial visit to last 60-75 minutes. We recommend wearing comfortable, loose-fitting clothes so your therapist can appropriately asses the area of pain or injury. Don’t worry, we’ve got changing facilities at each of our locations
Payment
Co-payments and/or co-percentages are collected at each physical therapy visit. We accept cash, checks, or credit card payments.
Bring your paperwork!
Sometimes it’s a dirty word, but not this time! Fill out the documents below before you come in for your evaluation – they help us get up to speed with what’s happening and will put you on the fast track to getting better.

- Medical History
- HIPAA Information Consent
- Medicare Questionnaire for Medicare patients only
- Workers Compensation Questionnaire for Workers Compensation patients only
- Auto/PIP Questionnaire for Automobile accidents or Personal Injury only
